Does the 11-hour rest rule between shifts work?
The one shift-design change with randomized-trial evidence behind it: keep at least 11 hours between the end of one shift and the start of the next.

The evidence in one line
A quick return, less than 11 hours between the end of one shift and the start of the next, is the single shift-design flaw with the strongest evidence against it. In a two-armed cluster randomized controlled trial across 66 hospital units and 1,314 workers, roughly halving quick returns reduced insomnia, sleepiness, and work-related fatigue (Djupedal et al., 2024). Separately, each additional hour of rest between two shifts was associated with about 5% lower injury risk (IRR 0.95, 95% CI 0.93-0.96) in a 69,200-worker Danish cohort.
The one intervention with randomized evidence
Most of what we know about shift work and health comes from cohort studies, which can show associations but cannot rule out that healthier people end up on easier schedules. The 11-hour rest rule is the rare exception. Djupedal and colleagues (2024) ran a two-armed cluster randomized controlled trial across 66 hospital units and 1,314 workers, assigning units to rosters that roughly halved the number of quick returns.
Because units were randomized, the improvement can be read as an effect of the schedule change rather than a quirk of who works which pattern. Halving quick returns reduced insomnia, sleepiness, and work-related fatigue. For a scheduler, that is a stronger warrant than almost anything else in this library: change the gap between shifts and the outcomes move with it.
The mechanism: lost sleep and rising injury risk
A field study helps explain why. Oster and colleagues (2024) tracked 90 nurses within their own schedules, comparing their sleep on quick returns against their sleep on ordinary day-to-day transitions. On quick returns, nurses slept 1.02 hours less by objective measurement (95% CI -1.23 to -0.81) and rated their sleep quality half a scale point lower (-0.49, 95% CI -0.69 to -0.31). Because the comparison is within the same person, it isolates the schedule from individual differences, though it remains observational rather than randomized.
The injury signal points the same direction. In a Danish register cohort of 69,200 workers, each additional hour of rest between two shifts was associated with about 5% lower injury risk, or equivalently roughly a 5% increase in injuries for each hour less between shifts (IRR 0.95, 95% CI 0.93-0.96; Nielsen et al., 2019, summarized in the Garde et al., 2020 expert consensus). The dose-response shape, more rest linked to fewer injuries at each step, is part of why experts treat the 11-hour gap as a floor rather than a target.
How to read the 11-hour threshold
Two cautions keep this honest. First, the 11-hour figure is the research definition of a quick return, not a magic number. The injury relationship is continuous, so a longer gap is better than a shorter one at every step. Treat 11 hours as the minimum you defend, not the rest you aim for.
Second, the strength of this evidence is about the intervention, not a promise of safety. Halving quick returns lowered fatigue and insomnia in the trial; it did not eliminate them, and no study here shows that an 11-hour gap makes night work safe. What the evidence does support is narrow and useful: of all the levers a scheduler controls, closing quick returns is the cheapest to implement and the best tested.
What this means for your schedule
- Set a hard minimum of 11 hours between the end of one shift and the start of the next, and treat any shorter gap as an exception that needs sign-off.
- Audit your current roster for quick returns first; the usual culprits are a late evening shift followed by an early morning, and swaps that quietly compress rest.
- When a tight turnaround is unavoidable, widen the gap as far as the roster allows rather than settling at the legal floor, since injury risk falls with each added hour of rest.
- Build the 11-hour check into whatever tool publishes the schedule, so violations are blocked before staff see them rather than caught afterward.
- Track the count of quick returns as a standing metric and drive it down, the same change that improved outcomes in the trial.
The business case
Closing quick returns is the lowest-cost health and safety intervention available to a scheduler: it changes the order of shifts, not headcount, budget, or pay.
It is also the best-evidenced. A randomized controlled trial, not correlation alone, links halving quick returns to lower fatigue and insomnia, and a 69,200-worker cohort links each added hour of rest to about 5% lower injury risk, the kind of exposure that drives incident costs and workers' compensation claims.
Because an 11-hour rest interval also appears in many working-time regulations and union agreements, enforcing it by default lowers compliance exposure at the same time.
